About The Role

The purpose of the role is to assist the Registered Manager and Service Delivery Lead to organise the day to day running of the service provision through the allocation of schedules to care staff, support staff and key workers. To provide verbal instruction and advice to care staff, support staff and key workers. on Care at Home(Wearside) Ltd policies and procedures. 

 

This role carries the authority that comes from being a senior member of the care team and being respected by junior staff. It is expected that the service coordinator will carry out administration tasks as delegated to them by the management team and be willing to train, coach and mentor junior staff as needed.  

 

The list of tasks suggested below is not exhaustive but illustrates the broad range of tasks and responsibilities that could fall within the remit of this role. 

It is the responsibility of the Care Coordinator to support the Registered Manager in achieving the aims and objectives of the Company and achieving the level of quality in the service provided in accordance with the Company’s Quality Assurance Policy. 

 

To assist the Service Delivery Lead/ Registered Manager in marketing the services provided by the Company and liaising with commissioners, prospective Service Users, health professionals and other agencies connected with the service. 

 

To be responsible to the Service Delivery Lead/ Registered Manager for the day to day service provision by producing detailed work schedules for the Care and Support Workers and providing both written and verbal instructions/guidance to them. 

To be conversant with the Care Standards Act 2008 for Domiciliary Care Regulations (National Minimum Standards) and legislation governing the service and other regulations concerning the provision of domiciliary care including the Health and Social Care Act 2008. 

To keep an accurate record of staff sickness in order to assist the HR team to utilise the Bradford factor and monitor sickness levels 

To be aware of the Quality Assurance Policy of the Company in the provision of a quality service to the Service Users. 

To represent the Company in a professional manner at all times, on the telephone, face to face or in written communication.  

To ensure that telephones are answered promptly, and people are spoken to in a polite and respectful manner. 

To maintain confidentiality at all times and carry out the Company’s Confidentiality Policy. 

To carry out the monitoring of the service in the field and liaise with users of the service, their advocates and professionals involved in their care and support to ensure that the service provision delivered meets the quality targets of the Company, and the contractual requirements of commissioners. 

To inform the Service Delivery Lead / Registered manager of the outcomes and issues that are identified in the monitoring of the service. 

To report to the Registered Manager any issues regarding the legislative requirements concerning the services provided and the staff providing them.  

To report to the Registered Manager any financial issues that have an impact on the overall budget.  

 To assist the Service Delivery Lead / Registered Manager in holding team meetings

To maintain accurate records of phone calls made and received and details of actions taken using the care management software. 

To manage the senior staff team and coordinate an out of hours rota, holding weekly handover meetings and updating client and staff schedules as appropriate.
